Health History & Emergency Contact Information:

Prior to the start of camp you will need to fill out a Camper Health History & Emergency Contact Form.  Please click the link below to access the form.  Also, the State of New Jersey requires that camper's immunization history is on file prior to the camper's participation.  A copy can either be uploaded to the form or emailed separately.

 

HEALTH HISTORY & EMERGENCY CONTACT FORM

 

Daily Check In and Check Out:

Check-In and Check-Out will take place at the parking lot adjacent to Spartan Turf Athletic Complex (see directions below). Check in will be between 9:00-9:15am and check out will be 11:30-11:45am for Half Day and 3:15-3:30pm for Full Day.  Staff will be on site by 8:45am for any early arrivals.

 

Late Arrivals: If camper is arriving late to camp, parents must be sure to check in their child with a camp staff member at the turf fields adjacent to the parking lot.

Early Departure: If a parent must pick-up their child before the camp's scheduled departure time a note must be left with the attendance desk at check in or an email must be sent.

​

Meals:

Lunch will be provided Monday thru Friday for our Full Day campers in the college's air conditioned cafeteria.  Daily selections include a variety of choices such as pasta, pizza, hamburgers, hot dogs, sandwiches, salad bar & special lunch of the day.  We suggest that campers come with a snack for the morning.  The camp will have water, gatorade and snacks on hand for purchase as well.

If your child has special dietary needs/restrictions please be sure to let us know in advance so we can accommodate his/her needs.

Campers should bring the following:

Refillable Water Bottle (clearly marked w/camper's name)

Soccer Ball

Cleats

Shinguards

Sneakers/Indoor Shoes

Rain Jacket

Extra Set of clothes (in case of rain)

The camp assumes no responsibility for misplacement or loss of personal items.  Campers are advised not to bring valuables to camp.

 

CELL PHONE USE DISCOURAGED DURING CAMP HOURS

 

In the event of an emergency a camp staff member will contact the parent/guardian.

Address: 280 Route 340, Sparkill, NY 10976

 

From New York City Via the George Washington Bridge:

Bear right crossing the bridge following signs to Palisades Interstate Parkway. Travel on the Palisades Parkway to Exit 5 (Sparkill - Route 303 North). Bear right coming off the ramp and go north on Route 303 to the SECOND traffic light. Turn right on Route 340. The College is approximately 1/2 mile on the left. Drive past college and the field is 1/3 mile on the right.

From Westchester or Connecticut via Tappan Zee Bridge:

Travel on the New York Thurway to Exit 12. Bear right coming off the ramp and travel south on Route 303 about 4 miles to Route 340. Turn left on Route 340. The College is about 1/2 mile on the left. Drive past college and the field is 1/3 mile on the right.

From Bergen County:

Follow directions as above via Palisades Interstate Parkway or take Livingston Street from Northvale which changes into Route 303, and follow as above.
