Health History & Emergency Contact Information:
Prior to the start of camp you will need to fill out a Camper Health History & Emergency Contact Form. Please click the link below to access the form. Also, the State of New Jersey requires that camper's immunization history is on file prior to the camper's participation. A copy can either be uploaded to the form or emailed separately.

DIRECTIONS
GPS ADDRESS - 505 Ramapo Valley Road, Mahwah, NJ
After pulling into the main entrance of the college, proceed to the security booth. Let them know you are checking in for soccer camp. After passing through the gate stay left and follow the road past two stop signs. After the second stop sign make your next right up the hill and you will see Overlook Hall on your right. Pull up around and you will find parking adjacent to the dorm.
